Please help us to ensure that our communications with you run as smoothly as possible by immediately informing us of any changes in your contact details.
If you keep a list of people to contact when you move house or change your personal details then please add us to it, it may also be a good idea to add the names of any hospitals, clinics, dentists, opticians, etc.
You should let us know immediately if you have:
- Changed your name
If you change your name by deed pole, through marriage / divorce or for religious or other reason you must let us know so that we can update your records.
If we do not have the change recorded, there is a chance that important medical data could be lost or misplaced as records would not follow from one name to the other. Also you may find that you are unable to access confidential personal information at the surgery as your identity change was not recorded.
If you are commonly known by another name or nickname (to friends or family for example) please let us know this also. It will help eliminate confusion. - Changed your permanent home address
If you change your address and don't let us know, the doctor won't know where to find you if you request urgent home visit.
When you let us know of a change of address please also let us know if that change affects anyone else who may be registered as a patient at the surgery and moving home with you. - Will be living at another address temporarily

If you arrange a telephone consultation with our doctors or nurses we could be calling the wrong number.
If we need to call you with urgent test or x-ray results we will not be able to do so. - Got a new mobile phone number
If you do not have a fixed line telephone or if you are not always at your home address, please let us know your mobile number.
Some NHS services now offer text reminders. We must have your number if you want to use these services. - Changed your mobile phone number
Please let us know if you change your mobile number. Some telephone companies re-use numbers after 3 months, so your results may be going to a complete stranger unless you keep us informed of every change. - Changed or got a new email address
If you have requested to be kept up to date with our Surgery Newsletter by email you should let us know if you are switching to a new email address. We unsubscribe all addresses after 2 failed delivery attempts.
